Effective integration with the community nursing team, reablement team, and GP practice
The service integrated with health and social care colleagues exceptionally well. Staff provided support in the community in conjunction with health and social care colleagues for example, staff were trained to carry out delegated health tasks from both the community nursing team and GP practice, which reduced the time people had to wait for some health interventions, such as compression stockings and inhalers.
The reablement support provided by staff to people was therapy led. Occupational therapy and physiotherapy teams supported staff to devise therapeutic reablement goals for people. Joint visits took place between staff and therapists. This meant that skills and knowledge were shared, and the ordering of low level equipment, meant that people did not have to wait for long for therapy.

One Devon: Devon ICS’ approach to integrating their workforce across the system
Devon Integrated Care System (ICS) established the One Devon partnership, a collaboration between the NHS, social care, local councils and voluntary communities, social enterprise sector and people who use services in the Devon community. The aim of this partnership is to assure that everyone in Devon has an equal chance to live long, healthy and happy lives. Prior to One Devon, the services across Devon were working in silos.
